Appointment Cancellation and Refund Policy

We want you to have a great experience at Simple Healthcare. We can only give you
that experience if you show up for your scheduled appointment!

You can cancel or reschedule your appointment online at your convenience through
following the link on the email you received, or please call us at (888) 674 2742.

Please understand that a no call/no show takes an appointment away from someone
who really wanted to come in and enjoy the exceptional care that goes into a visit at
Simple Healthcare.

Please give a 24-hour notice in order to receive a full refund. Any cancellations less
than 24 hours but greater than 12 hours are subject to a cancellation fee of 50% of the
cost of appointment. This includes half of the cost of the appointment and a restocking
fee. Any cancellations less than 12 hours will not receive a refund.

When you submit an appointment, you also agree to receive communications from
Simple Healthcare – we will never spam you or sell your information.

Links to Third-Party Sites/Services

www.helloshc.com may include links to other websites (“Linked Sites”). These Linked
Sites are not under the control of Simple Healthcare, and we are not responsible for
their content. Inclusion of these links does not imply our endorsement or association.

Class actions are not allowed.

Termination

Termination/Access Restriction Simple Healthcare reserves the right, in its sole

thereof at any time, without notice. To the maximum extent permitted by law, this
agreement is governed by the laws of the State of New York and you hereby consent to
the exclusive jurisdiction and venue of courts in New York in all disputes arising out of or
relating to the use of the Site. Use of the Site is unauthorized in any jurisdiction that
does not give effect to all provisions of these Terms, including, without limitation, this
section.

Simple Healthcare reserves the right, in its sole discretion, to change the Terms under
